This form gathers essential information, including:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\"><strong>Client Information<\/strong><\/span><\/li>\n<li><span style=\"color: #000000;\"><strong>Payment Type<\/strong> (e.g., Medicare, NDIS)<\/span><\/li>\n<li><span style=\"color: #000000;\"><strong>Informed Consent <\/strong>(which includes our Privacy, Appointment and Cancellation Policies)<\/span><\/li>\n<li><span style=\"color: #000000;\"><strong>Emergency Contact details<\/strong><\/span><\/li>\n<\/ul>\n<p><span style=\"color: #000000;\">Additionally, providing the following relevant paperwork before your initial meeting helps our allocation team and psychologist get a better overview of your concerns and needs:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\"><strong>Referral Letters<\/strong> from healthcare professionals (e.g., GP, Paediatrician, Psychiatrist) or schools.<\/span><\/li>\n<li><span style=\"color: #000000;\"><strong>Treatment Plans<\/strong>, such as:<\/span>\n<ul>\n<li><span style=\"color: #000000;\">GP Mental Healthcare Plan (MHCP)<\/span><\/li>\n<li><span style=\"color: #000000;\">GP Chronic Condition Management Plan (GPCCMP)<\/span><\/li>\n<li><span style=\"color: #000000;\">Psychiatrist or Paediatrician Complex Neurodevelopmental Disorder Treatment &amp; Management Plan (for individuals under 25 years old)<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\">Any <strong>reports from other health professionals<\/strong> or previous therapists.<\/span><\/li>\n<\/ul>\n<h3><span style=\"color: #372c25;\">NDIS Participants<\/span><\/h3>\n<p><span style=\"color: #000000;\">If you or your child is an NDIS participant, we require you to:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\">Complete an <strong>NDIS Service Agreement<\/strong>.<\/span><\/li>\n<li><span style=\"color: #000000;\">Provide your <strong>NDIS plan<\/strong> with funding allocation and goals.<\/span><\/li>\n<li><span style=\"color: #000000;\">Provide your <strong>Plan Manager&#8217;s details<\/strong> (if applicable).<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #000000;\">This information allows your psychologist to effectively plan your support, manage logistical timeframes, and address your specific goals.<\/span><\/p>\n<h3><span style=\"color: #372c25;\">Telehealth Consultations<\/span><\/h3>\n<p><span style=\"color: #000000;\">For Telehealth consultations, full payment is required <strong>two days prior<\/strong> to your scheduled appointment.<\/span><\/p>\n<h3><span style=\"color: #000000;\">Privacy<\/span><\/h3>\n<p><span style=\"color: #000000;\">You can review our comprehensive Privacy Policy, detailing how we securely store your confidential information, at: <a style=\"color: #000000;\" href=\"https:\/\/www.sprout-psychology.com.au\/privacy\">https:\/\/www.sprout-psychology.com.au\/privacy<\/a><\/span><\/p>\n<h3><span style=\"color: #000000;\">Referrals &amp; Medicare Rebates<\/span><\/h3>\n<p><span style=\"color: #000000;\">A referral from your GP is <strong>not a requirement<\/strong> to engage with our service. However, if you wish to claim a rebate under the Medicare Benefits Scheme, you <strong>must obtain a relevant treatment plan<\/strong> from your GP, Paediatrician, or Psychiatrist and provide us with correct Medicare Card details.<\/span><\/p>\n<p><span style=\"color: #000000;\"><strong>Important:<\/strong> You must obtain your referral and treatment plan(s) <strong>prior to attending your psychology appointment<\/strong>. Rebates cannot be backdated to cover sessions conducted before the plan was in place.<\/span><\/p>\n<h3><span style=\"color: #000000;\">Applicable Treatment Plans:<\/span><\/h3>\n<ol>\n<li><span style=\"color: #000000;\"><strong>Mental Healthcare Plan (MHCP):<\/strong> This plan is available to individuals diagnosed with a mental health condition. With an MHCP, you can receive rebates for up to 10 psychology sessions per calendar year.<\/span><\/li>\n<li><span style=\"color: #000000;\"><strong>GP Chronic Condition Management Plan (GPCCMP):<\/strong> Your GP completes this management plan for ongoing health conditions (present or likely to be present for 6 months or longer) to help improve health outcomes. With a GPCCMP, you can receive rebates for up to 5 individual services across one or more disciplines each calendar year.<\/span><\/li>\n<li><span style=\"color: #000000;\"><strong>Complex Neurodevelopmental Disorder Treatment &amp; Management Plan (Previously HCWA Plan):<\/strong> If you are under the care of a paediatrician or psychiatrist and are under 25 years of age, you may be eligible for a Medicare rebate through this plan. To qualify, you will need a consultation with the specialist and a referral letter specifically requesting an assessment for a complex neurodevelopmental disorder (e.g., Autism). You may be eligible for a lifetime total of:<\/span><\/li>\n<\/ol>\n<ol>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"color: #000000;\"><strong>4 assessment sessions<\/strong> \u2013 Please note that for a rebate to be provided, the service must be provided to the patient individually and in person; parent-only sessions are not claimable under this item.<\/span><\/li>\n<li><span style=\"color: #000000;\">Up to <strong>20 therapy sessions<\/strong> across disciplines.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">As of 2025, the current Medicare rebate is <strong>$98.95 per consultation<\/strong> under an MHCP or Complex Neurodevelopmental Disorder Treatment &amp; Management Plan, and <strong>$61.80 per consultation<\/strong> under a GPCCMP.<\/span><\/p>\n<p><span style=\"color: #000000;\">Rebates can only be processed from the date specified on the referral letter. We can typically process these rebates on your behalf; please indicate your preference on the Client Registration Form.<\/span><\/p>\n<h3><span style=\"color: #372c25;\">Please Note:<\/span><\/h3>\n<ul>\n<li><span style=\"color: #000000;\">Assessments are <strong>not eligible<\/strong> for Medicare rebates under the MHCP or GPCCMP.<\/span><\/li>\n<li><span style=\"color: #000000;\">Medicare requires the individual receiving the service to be present for their sessions. You <strong>cannot claim<\/strong> under another person&#8217;s Mental Healthcare Plan or Chronic Diseases Management Plan.<\/span><\/li>\n<li><span style=\"color: #000000;\">Parents of children are eligible for <strong>2 parent-only sessions<\/strong> per calendar year under an MHCP. Common service codes are P100 for initial therapy sessions and P200 for subsequent sessions.<\/span><\/p>\n<h3><span style=\"color: #372c25;\">Understanding Your Rebates<\/span><\/h3>\n<p><span style=\"color: #000000;\">We know that managing healthcare costs is important, and we want to help you make the most of any available rebates. Please note that it is your responsibility to ensure you have a valid and current referral and treatment plan, and to confirm your eligibility for Medicare rebates or Private Health Insurance claims. As these benefits are managed by external providers, we do not have control over your individual eligibility. We encourage you to verify all necessary details with Medicare or your private health insurer prior to your appointment to avoid any unexpected out-of-pocket costs.<\/span><\/p>\n<h3><span style=\"color: #372c25;\">Fees &amp; Payments<\/span><\/h3>\n<p><span style=\"color: #000000;\">The Australian Association of Psychologists Inc (AAPi) recommends a fee of $330 for a 45 to 60-minute consultation. Our current consultation fee ranges from <strong>$260 to $290 for a 50-minute session<\/strong>. You will receive a confirmation email with the exact fee schedule upon booking your first appointment.<\/span><\/p>\n<p><span style=\"color: #000000;\">Hourly rates apply for sessions running overtime (e.g., an additional 15 minutes incurs a charge for 0.25 hours). Services included in the standard consultation fee are outlined in the &#8220;Services Included in Consultation Fees&#8221; section below.<\/span><\/p>\n<p><span style=\"color: #000000;\"><strong>Available payment options include:<\/strong> EFTPOS, MasterCard, VISA, Direct Deposit, or Cash. Payments made with debit or credit cards will incur a small percentage surcharge, which is automatically deducted by the EFTPOS machine. This surcharge is non-refundable.<\/span><\/p>\n<p><span style=\"color: #000000;\">Clients are required to make full payment on the day of service, and no later than 14 days after the appointment.<\/span><\/p>\n<p><span style=\"color: #000000;\"><strong>For NDIS clients:<\/strong> We will provide you with an invoice to claim through your NDIS portal. If your plan is &#8220;Plan Managed,&#8221; please ensure you provide us with your Plan Manager&#8217;s email address so we can direct invoices to them.<\/span><\/p>\n<h3><span style=\"color: #372c25;\">Services Included in Consultation Fees<\/span><\/h3>\n<p><span style=\"color: #000000;\">To ensure best practice and comprehensive care, the following services are included in your consultation fees:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\">Brief emails and short phone calls to other health professionals or teachers (under 10 minutes).<\/span><\/li>\n<li><span style=\"color: #000000;\">Preparation and planning time for each session.<\/span><\/li>\n<li><span style=\"color: #000000;\">Clinical notes documentation for each session.<\/span><\/li>\n<li><span style=\"color: #000000;\">Medicare reporting (MHCP feedback letter back to your GP).<\/span><\/li>\n<li><span style=\"color: #000000;\">Resource development for sessions.<\/span><\/li>\n<li><span style=\"color: #000000;\">Research related to your care.<\/span><\/li>\n<\/ul>\n<h3><span style=\"color: #372c25;\">Additional Services (Incurring Extra Costs)<\/span><\/h3>\n<p><span style=\"color: #000000;\">The following services are offered at an additional hourly rate, as they fall outside of standard consultation inclusions:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\"><strong>Reports for Funding &amp; Applications:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">Preparation and completion of funding application forms or reports (e.g., Disability Support Pension Form, Carer Allowance Form, NDIS Access Request Form, School funding applications like KIS, DIP).<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\"><strong>Assessment Reports:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">Preparation and completion of reports for Cognitive, Academic, Autism, ADHD, and other psychological assessments (e.g., for TAC, WorkCover).<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\"><strong>Formal Correspondence:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">Preparation and completion of letters to NDIA, ART, other specialists, allied health professionals, or GPs.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\"><strong>School-Related Services:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">Preparation of classroom strategies, recommendations, or Individualized Education\/Learning Plans (IEP\/ILP).<\/span><\/li>\n<li><span style=\"color: #000000;\">School visits and school IEP\/ILP meetings.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\"><strong>Extended Professional Consultations:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">Phone consultations and discussions with teachers or other professionals exceeding 15 minutes in length.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\"><strong>Off-Site Consultation Costs:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">For off-site services (e.g., home visits, school visits), additional costs will apply. You will receive a quotation for the off-site service before proceeding. These costs include:<\/span>\n<ul>\n<li><span style=\"color: #000000;\">The <strong>psychologist&#8217;s labour time<\/strong> spent travelling to and from off-site services (e.g., home visits, school visits), and<\/span><\/li>\n<li><span style=\"color: #000000;\">The <strong>non-labour costs<\/strong> associated with off-site services, such as road tolls, fuel, and parking fees.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #000000;\"><strong>Extra Documentation and Administrative Tasks:<\/strong><\/span>\n<ul>\n<li><span style=\"color: #000000;\">This covers additional documentation and administrative efforts specifically for disability access purposes (e.g., to meet NDIS).<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3><span style=\"color: #372c25;\">Appointment Punctuality<\/span><\/h3>\n<p><span style=\"color: #000000;\">Please aim to arrive <strong>5 minutes before<\/strong> your scheduled appointment. Late attendance will shorten your consultation time. Sessions extended by more than 10 minutes past the scheduled end time may incur additional charges based on our hourly rate. However, if you wish to obtain a rebate under the Medicare Benefit Scheme, you are required to obtain a Mental Healthcare Plan (MHCP) from your GP PRIOR TO attending your first appointment. If you are eligible for a MHCP and have obtained one before engaging us, you might be able to claim $89.65 each consultation.<\/p>\n<p>If you have Private Health Insurance, please check with your policy prior to your engagement to check your eligibility and limit.<\/p>\n<p>Sprout Psychology has no control over your eligibility to receive rebates or health insurance and has no involvement with the process of obtaining a treatment plan.<\/p>\n<h3><span class=\"wixui-rich-text__text\">As<\/span>sessments and reports<\/h3>\n<p>Assessments and reports are charged separately and are inapplicable for Medicare rebates.<\/p>\n<p>Please be reminded, it is clients&#8217; personal responsibility to check their eligibility to obtain a Mental Healthcare Plan or Private Health Insurance.<\/p>\n<p>Please note you cannot claim another person\u2019s Mental Healthcare Plan or Chronic Diseases Management Plan as Medicare requires that the person must be present for their sessions.<\/p>\n<p>You also cannot claim two insurance at the same time.<\/p>\n<h3>Fees &amp; Payments<\/h3>\n<p>APS National Schedule of Recommended Fees recommends 45-to-60-minute consultation fee as $300. Please leave us with the Plan Manager&#8217;s email if your plan is &#8220;Plan Managed&#8221;.<\/p>\n<p>Please read the following carefully<\/p>\n<h3>Services Included in Consultation Fees<\/h3>\n<ul>\n<li>To ensure the delivery of best practice, the following services are included in the fees stated above:<\/li>\n<li>Emails and short phone calls to other health professionals\/ teachers (&lt; 10 minutes)<\/li>\n<li>Preparation and planning time for each session<\/li>\n<li>Clinical notes documentation for each session<\/li>\n<li>Medicare reporting (Mental Healthcare Plan and CDM reports back to your GP)<\/li>\n<li>Resource development for sessions<\/li>\n<li>Research<\/li>\n<\/ul>\n<p>The following are additional services that can be offered with additional costs based on hourly rate:<\/p>\n<ul>\n<li>Preparing and completion of funding reports<\/li>\n<li>Preparing and completion of reports of Cognitive, Academic assessments, ASD, ADHD assessments, and other psychological assessments (e.g., TAC, Workcover).<\/li>\n<li>Formal letters to specialists\/ GPs<\/li>\n<li>Completion of carer allowance forms<\/li>\n<li>Completion of NDIS Forms<\/li>\n<li>Completion of NDIS Reports<\/li>\n<li>Preparing classroom strategies \/ Individualized Learning Plan<\/li>\n<li>School Visits and school ILP meetings<\/li>\n<li>Teacher\/ professional phone consultations\/ discussions (more than 15 minutes in length)<\/li>\n<li>All additional services and report writing are billed at an hourly rate.<\/li>\n<\/ul>\n<h3>Travel\/Parking:<\/h3>\n<p>The clinic address is 369 Burwood Hwy, Burwood VIC 3125. Client Parking is available on site. The closest train station is Burwood Station. There are multiple bus lines and trams to the clinic. 75 tram line stops right in front of the practice.
